In order to provide a safe environment for all of our attending families, we require any ADULT that will be volunteering or fulfilling required Service Hours to complete this form.
MEDICAL RELEASE
Please complete 1 per student.

We cannot function properly without the help and support of our dedicated families. We know that sometimes there are not enough hours in the day to help outside of the home so we offer a Service Hour Opt-Out. If you need to opt-out, please submit this form along with your opt-out fee of $120 per semester.
