NEW FAMILIES,

So far you have printed the New Family Registration Documents and are working on collecting your supporting documents. All documents below need to be emailed in a Google Drive folder to us at registration@dragonflyhybrid.com.

- Copy of Original Birth Certificate

- Copy of State ID or Driver's License (ID address must match proof of residency documents)

- Proof of Residency #1 (current Mortgage Statement or Rental Lease Agreement)

- Proof of Residency #2 (current utility bill)

- Immunization Record or Vaccine Waiver (either: up-to-date immunizations or 2022 Waiver made out to Oxford Virtual Academy)

Once you have all of your supporting documents: Compile all documents as PDF's in a Google Drive folder and email to registration@dragonflyhybrid.com.
