- Up to 3 children; if registering more than 3 students, please print a second copy

2. Health Appraisal Form - Print 1 per student

3a. School of Choice (SOC) - Print 1 per student (2 page form)
*If a student has an IEP or 504, a copy MUST also be provided with this document. 

3b. Parental Notice - This form is only for families that do not reside in Oakland County AND that currently have or had an IEP/504 in a different county.

4. Transcript / Report Card Request - Print 1 per student
- If your student was homeschooled, you are still required to submit this form. For previous school, list "Homeschooled."

5. Affirmation of Prior Discipline - Print 1 per student

6. Record Request - Print 1 copy per student.

7. OVA Student Agreement - Print 1 per student.

8. Grade Placement Agreement - Print 1 per student.
